Program Purpose & Goal

 To protect the health of our community and prevent the spread of infectious disease.Program services are supported by Department of Health and Mental Hygiene.
 
Service Eligibility Criteria 

Vaccine for Children (VFC) Birth through 18 years of age

  1. Is covered by or enrolled in Medical Assistance
  2. Does not have health insurance
  3. Is Native American (American Indian) or Alaskan Native
  4. Has health insurance that does not cover (pay for) vaccines

Adult Vaccines

  1. Does not have health insurance
  2. Health insurance does not pay for the vaccine  

Flu Vaccine

  1. Medicare
  2. Self pay

 Immunizations for children and adults.

Service Hours

 Monday – Friday, 9 a.m. – 5 p.m.
Spanish speaking clients appointments available Tuesday and Thursday 1 p.m. – 5 p.m.

Fees/Charges & Type of payment  accepted:  Sliding Fee Scale and Free based on insurance and age
