Disparities
Reduction Demonstration Grant
FY 2014
The Department of Health and Mental Hygiene, Office of
Minority Health and Health Disparities (MHHD), would like to announce the
opportunity for community-based organizations to apply for two
Minority Health Disparities Reduction Demonstration Grant for fiscal year 2014.
The focus of the grants will be minority adult cardiovascular disease and
minority infant mortality. Funding is available for the following
jurisdictions:
Minority Adult Cardiovascular Disease
Baltimore City
Worcester County
Minority Infant Mortality
Anne Arundel County
Charles County
Montgomery County
Please review the following Request for Applications (RFA)
and supplemental documents for eligibility requirements and guidelines. Feel
free to share with other interested applicants located in these
jurisdictions.
The FY 2014 Minority Health Disparities Reduction
Demonstration Program Grant Application deadline has been extended to April
22nd no later than 3:30 pm.
Please note, the following sections in the RFA have been amended:
Section F- Program and Fiscal Reporting Requirements
(Minority Infant Mortality Grant Only)
Available Funds- A maximum of $116,666 is
available for each local Minority Infant Mortality Reduction Demonstration
Program. These funds are available on an annual basis depending upon
the appropriations by the Maryland General Assembly through June 30,
2014 and pending acceptable performance and continued budget
availability. A renewal application must be submitted and approved each
year.
Section H- Application Outline and Content:
The Format- The grant application should be no less than ten
(10) pages and no more than 15 pages long (not including budget pages,
appendices and written budget narrative justification), using 12 pt. font, 1.5
inch margins, double spaced, and each page numbered sequentially.
One original hard bound copy along with four
additional copies, and an electronic copy must be in the MHHD Office no
later than 3:30pm on April 22nd. Please email the electronic application
