DHMH is considering developing a chronic care medical home pilot to provide integrated care. Medicaid held a stakeholder meeting on the chronic care medical home pilot on Thursday, August 18, 2011. The power point presentation is available for your review. Based on this presentation, we are interested in your ideas on this potential pilot for Maryland.
